Memorandum of Agreement (MOA)

Do providers need to complete a Memorandum of Agreement (MOA)?

Yes, hospitals, critical access hospitals (CAHs), skilled nursing facilities (SNFs), and home health agencies (HHAs) are required to complete the MOA. If you have not yet completed the MOA, please do so as soon as possible. Please note: The MOA is the only agreement required between providers and Livanta. A Word version is available. You can also email a request for the electronic version to Livanta's Communications Team Lead.

Will Livanta sign a Business Association Agreement (BAA)?

No. As a QIO, Livanta is not a vendor to providers and therefore, the BAA is not applicable.
